PAYMENT POLICY

Medical services are provided and billed by our office. Our provider participates in most major health insurance plans. Following are the health plans which we are currently contracted with. If your plan is not listed, please call our office to verify.  Some insurances have out of network benefits so you still may be able to see our physician.

 

We offer Self Pay discounts for uninsured patients.

Deductibles and copays are due at the time of visit.  If you are unable to pay at your visit, please notify our practice manager prior to your visit so we can better assist you with payment options.
