Thank you for choosing Pham Cardiovascular Center for providing your cardiovascular care. In order to provide excellent medical care, please bring the following to your office visits:

• Current list of medications and vitamins

• Insurance Card and photo ID

• Co-Payment or other amounts due (as agreed upon by your insurance plan)

 

It is important to bring a list of medications with you to every visit to prevent duplicate medication prescriptions, screen for medication interactions, and prevent medication dosage errors.  A current medication list also assists coordinating medical care between your physicians.
